  • ALL athletes are required to have a FINALFORMS account operating at 100% which includes an up to date physical dated on or after April 15th, 2024 to participate in Preseason activities or Tryouts. Please see below for FINALFORMS Letter and Parent/Player Instructions.
  • Please see below for the Athletics Physical Packet. The packet must be completed, signed, and dated by a medical professional and the 1) Preparticipation Physical Exam and 2) Medical Eligibility Form MUST be uploaded onto FINALFORMS for full account approval.
