Important Information For Tryouts
ALL athletes will be required to have a FINALFORMS account operating at 100% which includes an up-to-date physical dated on or after April 15th, 2025 to tryout. Please see below for FINALFORMS Parent/Player Instructions, consent form, and sports physical forms (Medical Eligibility Form is the form needed to be filled out and uploaded to Final Forms.)
ALL athletes will be required to have 3 teacher or coach recommendation forms and consent form filled out and turned in by April 7th, 2026. Teachers are able to email me the recommendation form at [email protected].
